School Year Paperwork:
Please bring paperwork, shot records, care plans, etc. at the beginning of the school year. If I am not available, the front office staff will place paperwork in my mailbox. Faxing is not recommended during the summer because the machines may not be working. Medications and medication authorization forms can be brought to the clinic the week before school starts. Please feel free to call ahead to make sure the nurse or clinic assistant will be available if further details need to be discussed in person per parent.
Medications
All medication must be brought and picked up from the clinic by a parent/guardian. Children are not permitted to transport medications.
All medications require written permission from a parent/guardian.
Only prescription medication needed for the student to remain in school will be given by the clinic. Prescription medication must be in its original container and within date; it will be kept locked and stored in the nurse's office.
Over the counter medications will only be administered for a period of 2 weeks with written parent permission. A doctor’s note will be required for any over the counter medication needed longer than 2 weeks.
